Bufigen Nalbuphine
Nalbuphine is a morphine derivative that is used to relieve severe pain. It is used with anesthesia during surgery, for pain relief during childbirth. The drug may be addictive, so it can only be taken under the supervision of a doctor in a hospital setting.
Pharmacological action of Bufigen Nalbuphine
Nalbuphine hydrochloride is an opioid analgesic and an antagonist-agonist of opiate receptors. The action manifests itself in the disruption of interneuronal communication of the pain impulse. Nalbuphine acts at the level of the CNS and cerebral cortex, it has a sedative effect. It depresses respiration to a lesser extent than other opioid analgesics.
For quick action, the doctor prescribes the drug intravenously. The drug starts to work after a few minutes. When administered intramuscularly, it takes 10-15 minutes for pain relief. The action lasts for 3-6 hours. The maximum concentration of the active substance in the blood has been observed 30-60 minutes after administration. Semi elimination occurs after 3 hours. It is metabolized in the liver. Only a small amount has been excreted with urine.
Nalbuphine: dosage
Nalbuphine (Nubian) is given at a dosage of 0.15-0.3 mg per 1 kg body weight for pain relief. The dosage reaches 20-30 mg once in a vein for myocardial infarction. Premedication is 100-200 mcg/kg. Intravenous anesthesia is being administered at a dosage of 0.3-1 mg/kg slowly over 15 minutes.
Anesthesia maintenance is being performed under the supervision of an anesthesiologist at a dosage of 250-500 mcg/kg every 30 min through a catheter into the vein. During delivery, 20 mg is administered once intravenously into the gluteal muscle. Any doses adjustment is made only under the supervision of a physician.
Nalbuphine: schedule
Bufigen for pain relief should be administered at 4-6 hour intervals. In myocardial infarction, a second dose of 20 mg is allowed if the first dose did not produce the desired effect. If the patient has a history or is in the acute phase of drug dependence on morphine or its derivatives, 25% of the total dose is administered first.
The treating physician then monitors the patient’s response. If no withdrawal syndrome occurs, the remainder of the drug may be administered by the nurse. Caution should be exercised when prescribing the drug in elderly patients and if the respiratory system is impaired.
Nalbuphine: contraindications
Nalbuphine hydrochloride should not be used to treat patients who have been diagnosed with increased intracranial pressure or head injuries. The drug is contraindicated in respiratory function and CNS depression. Danger to health and life occurs with acute alcohol syndrome, alcoholic psychosis, and the use of Nalbuphine. Contraindications also include epilepsy.
The acute course of abdominal diseases that require surgical intervention may be treated with Nalbuphine with other drugs, only if an accurate diagnosis has already been made. During the use of Nalbuphine, symptoms of the pathology may be concealed or their intensity reduced. Also, during the medication, it becomes more difficult to detect the disease and make an accurate diagnosis. Morphine derivatives should not be prescribed if the patient is or has been addicted to these drugs.
Nalbuphine: side effects
Adverse reactions can occur in different body systems:
- respiratory
- GASTROINTESTINAL
- reproductive
- skin
More often, the side effects are a reaction to the mental state of the patient. It is not uncommon for medication dependence to occur. Patients under the influence of Nalbuphine (Nubain) should not drive vehicles, as the medication causes a pronounced sedative effect. Another side effect may be an allergic reaction, particularly anaphylactic shock.
The drug should be combined with caution with all agents affecting the CNS, because of the risk of depression. Sleeping pills, antidepressants, neuroleptics, and anxiolytics enhance the drug’s effect. Bufigen should not be combined with alcohol and other narcotic analgesics. Combination with Phenothiazine and Penicillin drugs may cause nausea and vomiting.
Nalbuphine in pregnancy
Nalbuphine Hydrochloride is contraindicated in pregnancy. The active ingredient passes through the placental barrier and breast milk. If the drug has been used during childbirth, it is necessary to monitor the newborn’s breathing. One adverse reaction is the suppression of respiratory function.
